Send the form or call. We'll put you on the book for a Monday through Thursday slot that fits around your run, and tell you anything you should bring.

Paperwork at the front desk
You fill out the federal health history form. It asks about medications, surgeries, and conditions a doctor is managing. Bring your list and this part takes five minutes.

The exam with Dr. Leal
Vision, hearing, blood pressure, a urinalysis dipstick, and a head-to-toe check. He'll ask about anything on your history that needs a closer look, and he'll tell you straight where you stand.

Card in hand, filed same day
If you're cleared, you leave with your certificate and we file it electronically with FMCSA and Texas DPS before the day is out. Nothing left for you to mail.
What's in the exam
The standard federal physical, nothing extra
This is the same FMCSA exam given anywhere in the country: vision and hearing, blood pressure and pulse, a urinalysis dipstick, and a physical check of your heart, lungs, spine, abdomen, and neurological function. There is no blood draw, and no X-ray unless something in the exam calls for one.
The urine sample is a health screen that looks for protein and sugar. It is not a drug test. A DOT drug screen is a separate service, and Dr. Leal is credentialed to collect it in the same visit if you need one.

How much does a DOT physical cost in Muleshoe?
$125 for the exam, paid at the front desk. No insurance is needed. If you also need a DOT drug screen, that is $50 more in the same visit, or $175 for both together.
Can a chiropractor perform a DOT physical?
Yes, as long as the chiropractor is certified and listed on the FMCSA National Registry of Certified Medical Examiners. Dr. Eli Leal is, under Registry #1054243805. It is the same federal exam and the same certificate you would receive from any other certified examiner.
How long does a DOT physical take?
Most drivers are finished in 15 to 20 minutes. Allow a little extra on a first visit for the health history form, or if you take medication Dr. Leal needs to review.
When do I get my medical card?
The same day. If you are cleared, you leave the clinic with your certificate in hand, and we file it electronically with FMCSA and the Texas Department of Public Safety before the office closes.
Do I need an appointment for a DOT physical?
It is best to have one, because the clinic runs Monday through Thursday, 8AM to 12PM and 2PM to 6PM, and closes Friday through Sunday. Send the form with the day you can come in, or call (806) 272-4000 and the front desk will find you a time.
Is the urine sample a drug test?
No. The urinalysis in a DOT physical is a dipstick health screen that checks for protein and sugar, which can point to kidney or blood sugar problems. A DOT drug screen is a separate, federally regulated test. Dr. Leal is credentialed to collect one in the same visit for $50 if your employer requires it.
Where do I park a truck and trailer?
There is open ground beside the clinic at 1106 W American Blvd with room for a full rig. American Boulevard is US-84 through Muleshoe, so you can pull in off the highway, walk in, and pull back out without dropping a trailer.
What happens if my blood pressure is high?
It gets rechecked before anything is decided. Blood pressure runs high on a lot of drivers who just spent four hours behind the wheel. If it is borderline, federal rules still allow certification on a shorter card while you get it managed, and Dr. Leal will walk you through what that means for your renewal date.
